Part I — Terms and Conditions of Service

Welcome to TeQuaidas Diagnostics ("Company," "we," "us," or "our"). By accessing our website (https://www.tequaidasdiagnostics.com) and utilizing our mobile phlebotomy, diagnostic support, and I-9 verification services, you ("Client," "User," or "Patient") agree to be bound by these Terms and Conditions.

1. Introduction and Scope of Service

TeQuaidas Diagnostics provides mobile phlebotomy, specimen collection (toxicology, infectious disease, chemistry), wellness screenings, and mobile/remote I-9 employment verification services. Services are currently provided in the Atlanta, GA metropolitan area and surrounding counties.

2. Medical Disclaimer (Not Medical Advice)

TeQuaidas Diagnostics is a mobile diagnostic support and specimen collection company. We do not provide medical advice, diagnosis, or treatment. Our services are limited to the collection of biological specimens and the facilitation of diagnostic testing ordered by authorized healthcare providers or requested by individuals via direct-pay models. All laboratory results are processed by third-party reference laboratories. In the event of a medical emergency, call 911 or visit the nearest emergency room.

3. Service Appointments and Booking

  • Booking: Appointments must be scheduled through our authorized platform.
  • Service area: Metropolitan Atlanta and surrounding Georgia counties. Out-of-area appointments may incur travel fees.
  • Accurate information: Users must provide accurate information, current location, and valid ID.
  • Access: Users must ensure a safe, clean, well-lit environment for our staff.

4. Cancellation and Refund Policy

  • Cancellation fee: A non-refundable $35.00 fee applies to appointments canceled within 24 hours.
  • No-shows: If our representative arrives and the patient is unreachable, the full service fee or the $35 fee (at our discretion) will be charged.
  • Refunds: Refunds for services already rendered are not permitted due to the nature of diagnostic collection.

5. I-9 Employment Verification

  • We offer both mobile (in-person) and live-video I-9 verification services.
  • The employer/employee is responsible for ensuring all original documents are present and valid at the time of verification.
  • We act as the authorized representative solely for completion of Form I-9; we do not provide legal or immigration advice.

6. User Responsibilities and Staff Safety

  • Provide a safe, clean, well-lit environment. We reserve the right to refuse service in unsafe or hostile environments.
  • Provide true and accurate personal and insurance information.
  • Restrain pets and maintain a smoke-free, non-hostile environment.

7. Limitation of Liability

TeQuaidas Diagnostics is not liable for delays in laboratory processing by third-party labs. To the maximum extent permitted by Georgia law, TeQuaidas Diagnostics shall not be liable for any indirect, incidental, or consequential damages. Our total liability for any claim shall not exceed the amount paid by the User for that specific service.

8. Governing Law

These terms are governed by the laws of the State of Georgia, without regard to conflict-of-law principles. Any legal action shall be brought exclusively in the courts located in Atlanta, Georgia.

Part II — Privacy Policy & HIPAA Compliance

1. Commitment to Privacy and HIPAA

TeQuaidas Diagnostics is committed to protecting your personal and Protected Health Information (PHI) in accordance with HIPAA. We use industry-standard encryption and secure CRM systems to manage your data.

2. Information We Collect

  • Identity data: Name, Date of Birth, Gender, SSN (for I-9 / employment purposes).
  • Contact data: Address, phone number, email.
  • Clinical data: Lab orders, specimen types, insurance information, and screening results.
  • Payment information: Processed via secure, encrypted third-party processors.
  • I-9 documentation: Sensitive identification documents (passports, driver's licenses, SSNs) as required for employment verification.

3. Sharing of Information with Third Parties

We do not sell your data. We share necessary clinical information only with authorized reference laboratories and partners to fulfill your service, including FAIS (Forensic Analysis & Investigative Services), UFC Lab, Boston Heart Diagnostics, and other authorized healthcare providers specified in your lab order. We may disclose information if required by law, subpoena, or public-health reporting. Mobile phone numbers and SMS consent data are never shared with third parties for marketing.

4. Data Security

We use industry-standard encryption and secure servers. Access is restricted to authorized personnel who need the data to perform their professional duties.

5. Patient Rights

  • Access your health records and request copies of your results.
  • Request amendments to incorrect information.
  • Request an accounting of disclosures of your PHI.

6. Cookies and Website Tracking
